Eligibility and Registration
ELIGIBILITY REQUIREMENTS
All children are admitted conditionally on a two (2) month trial period.
The director reserves the right to refuse admission or dismiss any child
whose behavior is disruptive or harmful to other children. Enrollment
in Faith Academy is open to all students who are able to perform the
academic work, meet the financial obligations, and agree to be
governed by the rules stated herein. At this time, Faith Academy is not
set up for and is not able to accept children with mental, physical,
emotional, health, or behavioral problems that require special facilities
or attention. If a child is dismissed there will be no refund of fees.
REGISTRATION PROCEDURE
The process of registration at Faith Academy is as follows:
1. Fill out appropriate forms.
a. Registration application
b. Permission forms
2. Pay a registration fee of $50 (non-refundable) & 1st weeks tuitions.
3. Complete a student and parent interview with director.
4. Pay a materials fee of $50.00 (SEPTEMBER)
5. Submit the following forms:
a. Immunization certificate (HRS Form 680)
b. Birth Certificate
c. Physical (HRS Form 3040)
